What constitutes a batter being called out on contact, one foot completely out of box, two feet completely out of box, fair or foul make a difference, and who's call is it????

One foot, completely out of the box at contact. Doesn't matter fair or foul.

One foot, completely out of the box at contact AND on the ground...

That is right you can be slapping a ball and one foot is all the way out of the box but not touching the ground and the other foot is somewhere in the box. Any time you have a foot all the way out of the box and touching the ground you are out.
