High school - never seen it.
14U - Yes. Younger smaller team getting a rally going and pitcher who had impeccable control threw ball at the helmet of batter. Caused a concussion.
14U Nationals - An Atlanta team scouted our team. Our best hitter hit 2 out of the park. When the Atlanta team faced us, pitcher looked over to coach, threw change up into thigh of our best batter. The pitcher and coach exchanged a smile after the pitch.
18U never seen it.

Stealing signals - every age group

Rolled and shaved bats - more prevalent than you would ever believe. Comes from the coed and beer softball league dads. One centex high school team - 6 players share the same old bat. Pop like you have never seen. I would guess 20% of high school bats. 10% of select play bats. If a player gets hurt from one of these, umpires often seize the bat. Could lead to sanction and ban even if the player did not know the parents did it.

So cheating is alive and well is softball. Pretty sad because when those kids hit .100 for their college team, they are pretty well benched and taken off scholarship. Many examples of this if you look carefully. Gold to college should not drop a player .300 points in the average. It's the bat.
